Coordinated Entry Specialist - Supportive Services

  • Conducting client intake and assessments using the standard CES assessments.
  • Entering and maintaining accurate data in the Homeless Management Information System (HMIS).
  • Coordinating referrals to housing and supportive services in collaboration with local service providers.
  • Monitoring client progress, conducting follow-ups, and updating records as needed to ensure continuity of care.
  • Serving as a liaison between community agencies, shelters, housing providers, and other CES partners.
  • Supporting case conferencing and prioritization meetings as required.
  • Assisting with CES data reporting and analysis for funding agencies and stakeholders.
  • Participating in training, community meetings, and regional coordinated entry planning.
  • Maintaining up-to-date knowledge of housing resources and community support programs.
  • Other duties as assigned.

Parent Resource Navigator - Brooke County

  • Support families of all types “where they are” - physically, mentally, and emotionally - including foster, adoptive, kinship, and grandfamilies
  • Provide community-based opportunities for parent/caregiver education, parenting classes, and supports
  • Working with the Family Support Center Director and/or Case Manager, meet with parents for initial assessment & ongoing referral to formal and informal community resources to enhance family function
  • Empower families through education and support to enable them to take the lead role in planning for and responding to their family's needs
  • Enhance parent's capacity by supporting them to build their protective factors and coping mechanisms
  • Work with families already involved with the Court/CPS system
  • Coordinate with the family, community partners, the court, and Child Protective Services, to strengthen and provide support in order to reach sustained reunification and additional parenting plan/family goals
  • Record quantitative and qualitative data on an ongoing basis for use in reporting and for continuous quality improvement
  • Attend trainings, meetings, and professional development opportunities associated with grant and programming requirements
  • Other duties as assigned.
